Problem with chapter numbers in PDF links

We recently found a problem in a PDF file that we created with the Mac 
versions of FrameMaker 6 and Acrobat Distiller 5.0. The source 
documents contain a lot of cross-references that convert to PDF links, 
such as:

      See “Defining passwords,” on page 9-1

In some but not all cases, the chapter number in the PDF link is 
incorrect, even though the book file "updated" properly with no reports 
of unresolved cross references, and the links all "jump" to their 
correct locations. The PDF often contains several links to the same 
chapter, and in some cases the chapter number is correct, and in other 
cases the chapter number is wrong.

The printed manual created at the same time contains the correct 
chapter numbers.

It seems as if something went wrong during the PDF creation process 
that caused the chapter numbers to change. Has anyone ever heard of 
anything like this occurring?
