No, you need to turn it on for each file. I believe it's off because
it adds some bulk.

I think comments are implemented using a layer, and more over it's a layer that
can be saved out independently. I don't think it has a lot to do with
PS, other than
work with it in a compatible way.

Yes, something. The layer that carries the comments, as well as an internal code
that is required to support them.

The comments have to be linked to locations and both the
core file and the additions saved in a binary format.
