Distiller Warning...

I used "Save as PDF" to make a PDF from a FM doc. The doc is 377 pages   
long and distilled okay except for the following warning after the last   
page:

% [ Warning: The following Named Destinations were defined more than once   
] %
       P.1
       P.10
       P.11
       P.12
       P.13
..etc.

The PDF is okay except that bookmarks were not created for the first 10   
(of 18) chapters and hyperlinks for these chapters from the ToC do not   
work.

As far as I can tell, the FM document is okay... anyone have a guess as   
to what is going wrong?
